Highnesses & their Household by any deletion title /deletion title his royal Highness pleases underline Junior to Dr. Warren /underline. By this mark of royal favour neither the Duke or Dutchess are presumed by me to be at all confined to me as their Physician (for that woud be absurd indeed) but I hope they would not suffer in any respect by that rank being conferred upon me to support me with the public & prevent me being superseded underline even nominally /underline by another obtaining a legal authority for that business which in fact I might be doing. I am here presuming that my attendance has not been unsatisfactory to their royal Highnesses; for if it has I can not reasonably hope for success in this application.
